File a Grievance

Basin Clinic's staff is here to assist with any issues or concerns our patient's and their family may have. Many concerns or complaints can be resolved quickly by contacting a member of the staff. If a staff members is unable to resolve an issue or address your concerns you can file a grievance below.

When submitting your grievance it is very helpful if you provide as much detail about the issue/concern as possible. Please include:

• Patient name and date of birth.
• Date of appointment or date of when the issue first started.
• Details regarding your grievance or concern.
• Your name, if different from the patient's name
• Your contact information.
• An thorough investigation will be conducted. In most cases, we are able to complete our investagation with 7 working days. If a medical chart review or other more detailed investagation is warrented you will be notified and given an approimate date the investigation is expected to be completed. 
• If you are not satisfied with our attempts to resolve your complaint you may request the complaint to be brought before the Board of Directors. You will be notified of the day and time of the next board meeting. Your attendance will be required!
